Social Work Services
Veritable offers social work assessment and interventions, with a speciality in supporting people with complex needs, including people receiving involuntary or legislated services, under guardianship orders, and/or with supported decision making needs.
Social work assessments and interventions help individuals and families to draw upon their strengths, identify strategies to improve their lives and achieve their goals. Our social workers can help carers and family members cope with challenging issues, and support people to identify the best way forward for themselves and/or their loved ones.
Veritable’s social work practice framework is humanist and strengths-based.
Social work services are particularly beneficial when the client needs are complex, there are multiple services involved, or where the person is refusing services but presents with risks to themselves or others. Social work can include initial assessment reports, recommendations for further support, individual and family engagement to identify solutions, stakeholder mapping, ecomaps and/or genograms.
These services can assist other service providers, including legal guardians and carers, to better understand the individual needs of a person so that their services can be more effective. Social work assessments and interventions are provided by our team of accredited social workers. Our services are available in Alice Springs (Mparntwe), Darwin and remote Aboriginal communities.
In some circumstances, for NDIS participants, social work services could include Specialist Coordination of Supports (COS). Specialist COS assists participants with complex needs to navigate their support services better and to identify the best providers and service mix for their needs. Specialist COS is sometimes required when participants have many funded and mainstream providers involved in their care and support, including government services.
Please note that Veritable does not offer ‘Level 2’ COS. In order to help individuals to find their right Level 2 COS, you can follow this link to a short video on how to choose your COS. This is one of the most important decisions you will make!
