Social Work Services
Veritable offers social work assessment and interventions, with a speciality in supporting people with disability, in aged care and older people living at home.
Social work assessments and interventions help individuals and families to draw upon their strengths, identify strategies to improve their lives and achieve their goals. Our social workers can help carers and family members cope with challenging issues, and support people to identify the best way forward for themselves and/or their loved ones.
Veritable’s social work practice framework is humanist and strengths-based.
Disability social work services are particularly beneficial when NDIS participant needs are complex, the evidence base for supports requires more detail and/or where the participant is refusing services but presents with risks to themselves or others. Disability social work can include initial assessment reports, recommendations, individual and family engagement to identify solutions, stakeholder mapping, and/or genograms.
These services can assist other service providers – such as support coordinators, guardians and carers – to better understand the individual needs of a person so that their services can be more effective. Social work assessments and interventions are provided by our team of accredited social workers. Our services are available in Alice Springs (Mparntwe), Darwin and remote Aboriginal communities.
Social work services also includes Specialist Coordination of Supports (COS) if required. Specialist COS assists participants with complex needs to navigate their support services better and to identify the best providers and service mix for their needs. Specialist COS is sometimes required when participants have many funded and mainstream providers involved in their care and support, including government services.
